Guidelines for the preoperative period

  • 1) PERFORM PREOPERATIVE EXAMS AS REQUESTED
  • 2) AUTHORIZE SURGERY. AFTER AUTHORIZATION IS COMPLETE, CONTACT THE SECRETARIAT OF THE ARTICULAR CLINIC.
  • 3) ENSURE FASTING DURING THE 8 HOURS PRIOR THE SURGERY SCHEDULE. DO NOT EAT OR DRINK WATER DURING THIS PERIOD.
  • 4) ARRIVE AT THE HOSPITAL AT THE INFORMED TIME, USUALLY 2 HOURS BEFORE SURGERY.
  • 5) BEFORE HOSPITALIZATION, INFORM IF YOU HAVE ANY INFECTION, EITHER A FLU OR ANY RESPIRATORY, URINARY AND SKIN INFECTION;
  • 6) IN CASE OF REGULAR USE OF MEDICATION, IT SHOULD BE TAKEN NORMALLY WITH A SMALL DOSE OF WATER, UNLESS THE DOCTOR SUSPENDS IT IN THE PREOPERATIVE PERIOD.
  • 7) KEEP ANTIHYPERTENSIVE DRUGS AND DRUGS FOR THYROID. AVOID VITAMIN SUPPLEMENTS OR GINGO-BILOBA OR TEAS.
  • 8) SUSPEND MEDICATIONS FOR DIABETES ON THE DAY OF SURGERY.
  • 9) ALWAYS INFORM THE DOCTOR IN CASE OF TREATMENT OF PREVIOUS DISEASES AND CHRONIC USE OF MEDICATION (MAINLY USE OF ANTICOAGULANTS, ANTIPLATELET AGENTS AND ANTI-INFLAMMATORIES).
  • 10) BRING ALL YOUR EXAMS REPORTS TO THE HOSPITAL EVEN IF THE DOCTOR HAS ALREADY SEEN THEM (FOR EXAMPLE, X-RAY IMAGES, ULTRASONOGRAPHY, CT SCAN, MRI, ELECTRONEUROMYOGRAPHY AND LABORATORY TESTS).
  • 11) ALWAYS BRING A COMPANION WHEN YOU COME TO THE HOSPITAL FOR SURGERY.

Guidelines for the postoperative period

  • 1) OPEN AND CLOSE THE FINGERS OF THE HAND CONSTANTLY, AVOIDING TO MOVE THE OPERATED FINGER.
  • 2) THE FIRST DRESSING SHOULD BE CHANGED ONLY BY THE DOCTOR, IN THE OFFICE OR HOSPITAL.
  • 3) AT BEDTIME: KEEP YOUR HAND ELEVATED WITH PILLOWS (TO KEEP IT ABOVE THE BODY LINE COMFORTABLY).
  • 4) DO NOT WET THE DRESSING (USE PLASTIC BAGS TO PROTECT AT BATH TIME).
  • 5) USE MEDICATIONS AS PRESCRIBED.
  • 6) DO NOT USE THE SLING ALL THE TIME. MOBILIZE THE SHOULDER AND KEEP YOUR HAND RAISED WHENEVER POSSIBLE.
  • 7) IF YOU NEED TO WASH THE WOUND, DO IT UNDER RUNNING WATER AND LEAVE THE WOUND DRY. DO NOT USE ANY TYPE OF SOAP, DETERGENT, OINTMENT OR ANY OTHER SUBSTANCE. DO NOT LEAVE THE WOUND MOIST.
  • 8)USE THE SPLINT ACCORDING TO MEDICAL ADVICE.

First aid - Amputation

Guidelines for prehospital conduct in case of limb amputation

1.Cleaning

  • » Wash the amputation stump with water or 0.9% saline solution and cover with discreetly compressing padding.
  • » Keep the limb elevated so it does not bleed.
  • » Avoid the use of tourniquets in the limb and the clamping of bleeding vessels.
  • » Then wash the amputated segment in the same way.

2.packaging

  • » Wrap the amputated segment with a gauze or any fabric available and put it in a plastic bag.
  • » Seal the bag tightly.
  • » Do not introduce any kind of substance (water, chemical or antiseptic) into the bag.

3.Conservation

Conservação
  • » Place the plastic bag in a styrofoam box containing ice and water.
  • » Avoid contact with the ice and part amputated. Remember that pure or dry ice in direct contact with the tissue can lead to irreversible damage.
  • » The 4 degree C environment is ideal for preserving an amputated segment that can range from 6 to 24 hours depending on the level of amputation.

4.Location for the specialized service

  • » Once you have completed steps 1, 2 and 3, immediately go to a specialist hand surgery service.
  • » The specialized team will make a careful evaluation of the condition of patient (age, profession, clinical condition) and of the amputated limb (time of ischemia, injury mechanism) to decide whether or not to replant.
